Plan Early


Reduce your child’s chances of being readmitted and increase the chances for a healthy recovery by planning early. Take steps as soon as possible during your child’s stay to plan for a successful transition from the hospital.


To begin, ask to speak with your nurse, and review the following:

 

  • discharge summary and discharge plan
  • complete medicine list and instructions
  • upcoming appointments
  • what to do if your child doesn’t feel well

Concerns About Follow-Up Care

Are you worried about being able to get the follow-up care your child needs? This could be having a home to go to, getting to an appointment, having money to pay for medicine, etc. To find out about resources that can help, contact our social work team at 651-229-3855.

Getting Back to Normal

After a hospital stay, your child may go through a period of readjustment at home. You may notice your child is acting out, clinging or has new fears. Try to be patient and understanding, but also be prepared to set normal limits on behavior and family rules. If your child’s behavior doesn’t stop or gets worse, contact your doctor for help.

Not Ready to Leave?

You have the right to appeal your child’s discharge if you don’t agree with the decision that they are ready to leave the hospital. Speak with your child’s doctor or nurse, and share your concerns. You may also need to reach out to Medicaid or your insurance company.

Repeat back what you hear the care team member say to make sure you understand the details correctly.
